Formulary

The Medica Formulary contains a list of generic and brand name prescription medications. These medications are covered at the co-payment level based on your health plan.

If your prescribed drug is on the formulary, and you go to a participating pharmacy, the prescription will be filled at the price of your co-payment. Your doctor will determine if a generic drug, which will have the lowest co-payment, is best.
